Step two - Contact Hawk-I to
check eligibility and apply.
You can call them at
1-800-257-8563 Monday through
Friday 8 AM to 7 PM.
Or
Go to their web site
(http://www.hawk-i.org/) to gather
additional information and either
download or fill out an application
on line.
Step three - Wait for a response to
your application. (Usually in within
two weeks)
Step four - When your child(ren)
has insurance in force call the
ESC (Employee Service Center)
at 1-888-539-2372 and tell them
you would like to remove your
children from you policy and
convert to whatever is appropriate
(single coverage or couples
coverage). They will also want to
know what insurance you have for
the child(ren). This can also be
done on the portal.
